How can I get a copy of my medical records?

A copy of your medical records can only be obtained through a written authorization. You may obtain an authorization for release of privileged information form by clicking here or by calling the Medical Records Department at (309) 543-8480. The authorization must be signed and dated by: the patient not the spouse, a guardian if the patient is a minor or under 18 years of age, the next of kin if the patient is deceased, a guardian if the patient is legally judged incompetent, or the emancipated minor. The consent must then be delivered to the Medical Records Department.
