We believe the relationship between the provider and the patient is very special and requires mutual trust. Confidentiality is of the utmost importance to our staff. The highest quality of care is possible when a provider and patient establish an open relationship with one another.
If a scheduled appointment cannot be kept, please let the office know as soon as possible. At that time an appointment can be rescheduled for a more convenient time.
Please arrive early for appointments as this will allow plenty of time to complete paperwork prior to seeing the provider. Bring a current insurance card and photo ID to the scheduled appointment.
Payment is expected at the time of service. Arrangements for payment can be made. Ask the receptionist for more information.
The Patient Portal is a great resource to view diagnostic testing, clinic notes and communicate with the provider.
