Nurse Supervisor – Havana/Mason City/Manito Medical Associates

Hours of Work: Monday- Friday, 8:00am -4:30pm (Full-time)

Qualifications:

Graduation from an accredited school of registered nursing is required.  Valid Illinois state license in nursing and CPR certification is required. Demonstrated outstanding ability to react calmly and effectively in emergent situations is required with the ability to prioritize and multi-task. Requires excellent organizational skills, oral and written communication skills. Completion of a bachelor’s degree in nursing is preferred. Previous leadership experience is preferred. Minimum three to five years physician office experience preferred.

Responsibilities:

Provide and facilitate nursing care to clinic patients and serve as the supervisor to address the needs of the clinic and facility. Responsible for educating clinic staff, ensuring nursing competency, onboarding of new nursing staff, education regarding quality measures and quality improvement projects. Ensure clinic staff know how to properly document to capture the required data to analyze and report performance. Assist in the evaluation of clinic staff and work with staff to continually improve performance. Identify and assist Clinic Director with clinic staff issues. Provide oversight of staff assigned daily responsibilities. Work with Medical Associates – EMR Specialists and EMR vendor to utilize the EMR system to the highest capacity and report any clinic or system issues. Oversee the implementation, management, and ongoing oversight of the immunization program. Reports to Clinic Director as appropriate.


 

MAINTENANCE MECHANIC I

Hours of Work: Monday- Friday, 7:30am -4:00pm, Full-time (Every third weekend and on-call required)

Qualifications:

  • Must be a self-starter, previous experience preferred but not required.
  • On the job training provided for lesser qualified individuals.
  • General job duties include but are not limited to: General maintenance, patching and painting, assisting experienced maintenance staff with tasks, assisting in construction projects, mowing and groundskeeping
  • Applicant must be able to rotate weekends every 3rd weekend and be on-call if and when on-call work requires.
  • Must have valid Illinois Driver’s License, pass a post offer drug screening, and be in good physical health.
  • Applicant must be able to read and follow directions for all types of work

Responsibilities:

  • Preventative Maintenance on most hospital equipment
  • Work Order requests for various jobs submitted by different departments.
  • Groundskeeping
  • Snow removal
  • Be familiar with all departmental policies and procedures

Employee and Community Health Education RN

Hours of Work: 40 hours per week, day shift, days/hours may vary (Full-time)

Qualifications:

  • Registered Nurse with current state Illinois license.
  • Bachelor of Science in Nursing required.
  • At least 5 years of healthcare experience required.
  • CPR/BLS certification required.
  • Knowledge of Employee Health for a hospital or healthcare organization.
  • Experience in leading multidisciplinary teams in a healthcare setting preferred.
  • Highly motivated with strong written and verbal communication skills.
  • Detail-oriented with strong analytical and problem-solving skills.
  • Ability to adapt to changing situations and workflows.

Responsibilities:

  • Works closely with Human Resources and Infection Control for Employee Health.
  • Maintains employee health records and administers immunizations.
  • Maintains up-to-date knowledge of healthcare employee education requirements and maintains annual employee education platform, Relias.
  • Works with ACLS, PALS, and BLS instructor(s) to coordinate class offerings to employees and the community.
  • Collaborates with the healthcare team at Mason District Hospital and community organizations to determine employee and community health and education needs.
  • Develops and maintains a nursing education program and competencies hospital-wide.
  • Reports to the Director of Quality, Infection Prevention, and Employee Health.

Surgical Scrub/Central Sterilizing Tech

Hours of Work: Monday-Friday (flexible to meet surgery schedule) (Full-time)

Responsibilities: A highly talented Surgical Technologist with experience assisting in operations, under the supervision of surgeons, registered nurses, or other surgical personnel; helping to set up the operating room, preparing and transporting patients for surgery, adjusting lights and equipment, passing instruments and other supplies to surgeons and surgeon’s assistants, holding retractors, cutting sutures, and helping to count sponges, needles, supplies, and instruments.

Qualifications:

  • High school graduate or equivalent
  • Scrub technician training program and certified in Illinois or at least 1 year on-the-job experience performing scrub tech duties
  • CPR certificate required

Emergency Department Tech

Hours of Work: 6:00pm-6:30am (Full-time with weekends)

Qualifications:

  • Must have valid Illinois EMT-B or CNA certification and valid CPR card.
  • Able to maintain professionalism and deliver high quality patient care during emergent situations.
  • Participate in patient centered care, with a team-based approach.
  • Must have strong clerical skills or the willingness to learn.

Hours of Work: Varies; Monday- Friday 8am to 6pm, every other Saturday 8am to 12pm (Full-time)

Qualifications: This position is 40 hour per week in the Havana Medical Associates Clinic. Medical office and computer experience required.  Electronic medical records experience preferred.  Must possess a positive attitude.  Exceptional patient relations experience preferred.  Must have the ability to multi-task and work as part of a team.  High school diploma or equivalent required.

PHYSICAL THERAPIST

Hours of Work: 8:00 A.M. TO 4:30 P.M. Monday through Friday and weekends as needed on a rotation basis. (Full-time)

QUALIFICATIONS/RESPONSIBILITIES:   

Licensed as a Physical Therapist by the State of Illinois.  Applicants must be a graduate from an accredited P.T. program with a minimum of a Doctorate Degree in P.T. and must possess an Illinois license or be eligible. The Physical Therapist is responsible for providing highly skilled evaluations and patient care for in-patients, out-patients, swing-bed, early intervention (optional) and home health clients.  Physical Therapy clients may include infants, toddlers, preschoolers, adolescents, adults, and geriatric population.  Applicants must be highly motivated, experienced, and professional with the ability to start programs, be a mentor, and work independently in the inpatient, outpatient, early intervention and home health settings.  One must have a current and valid Illinois Driver’s License, reliable transportation, and car insurance is required.   One must possess good handwriting, excellent communication skills, good computer operation skills, and operation of patient care equipment.


Registered Nurse- Med/Surg Department

Hours of Work:  Day & Night Shifts available, 6:30am- 7:00pm & 6:30pm-7:00am (Full-time)

JOB SUMMARY:  A Registered Nurse assumes responsibility and accountability for a set of assigned patients for a designated period of time.  During the designated time the RN will incorporate caring for all patients in all settings, and includes, but is not limited to, all of the following: A comprehensive nursing assessment every shift and as needed, the development of a plan of nursing care to be integrated within the patient-centered health care plan that establishes nursing diagnoses, and setting goals to meet identified health care needs, determining nursing interventions, and implementation of nursing care through the execution of nursing strategies and regimens ordered or prescribed by authorized healthcare professionals.  The Registered Nurse will administer medication in a timely manner.  The nurse will delegate nursing interventions to comply with the patient’s current plan of care and advocate for assigned patients.  The Registered Nurse will evaluate responses to interventions and the effectiveness of the plan of care. The nurse must communicate and collaborate with other health care professionals as needed to ensure continuity of care. The nurse will be expected to participate in development of policies and procedures, and systems to support patient safety.

Qualifications: 

  • Graduated from an accredited Professional School of Nursing
  • Current Illinois License.
  • Willing to obtain ACLS, PALS, and/or TNS after 1 year of nursing unless PRN
  • Must be willing to cross train to other departments within MDH
  • Compassionate, understanding self-motivated and enjoys working with others.
  • Demonstrates competency in nursing skills.
  • Desire to expand current medical knowledge.
  • CPR certification required
  • Able to handle stressful situations.

Senior Advantage–Licensed Clinical Social Worker—Outpatient Therapist

Hours:  Monday- Friday, 8:00am-4:30pm (Full-time)

Requirements:

  • LCSW is required, Master’s degree in Mental Health, Counseling, Social Work or Psychology
  • Illinois licensure

Qualifications:
Must have valid EMT, AMET, or Illinois Paramedic license.
ACLS, PALS, and PHTLS (or ITLS) required for AEMT and Paramedic applicants.
CPR required for all applicants.

Job Duties: The successful candidate will be responsible for treating ill and injured patients of all age groups. The EMT or Paramedic will perform skilled work while operating in an approved mobile medical unit, rescuing victims and performing advanced emergency care including the administration of life sustaining treatment to patients on-site and while in route to hospital. The candidate will be an active team member in the Emergency Department and be a positive influence in the organization.


Registered Nurse- Emergency Department

Hours of Work: PRN (Per Diem)

Qualifications:
Must have valid Illinois Registered Nurse Licensure.
ACLS and PALS required
TNCC or TNS preferred or willing to obtain
Minimum of 1 year of experience in nursing preferred
