If you’d like a copy of your medical records, you’ll need to fill out a Release of Information form. You can get the form by clicking here or by contacting our Medical Records Department:

The form should be signed by:
- The patient
- A parent or guardian if the patient is under 18
- The next of kin if the patient has passed away
- A legal guardian if the patient cannot make decisions
- An emancipated minor
Once completed, you can mail, fax, or bring the form to our Medical Records Department. We’ll handle the rest and make it easy for you.
