Submitting a FOIA Request

Requests for public records should be submitted in writing and should clearly describe the records being requested to help MDH respond efficiently. Requests may be submitted by mail or email to the FOIA Officer listed below.

Please include your name, contact information, and a description of the records requested. Applicable exemptions, including those protecting confidential patient information and other records exempt from disclosure under state or federal law, will be observed.

MDH may assess fees for copying records as permitted by law. If a request is denied in whole or in part, the response will identify the applicable legal basis for the denial and provide information regarding the requester’s right to seek review.
