Thank you for choosing Mason District Hospital Rehabilitation Services, Physical Therapy, Occupational Therapy, or Speech Therapy for your treatment. Please fill out the forms prior to your first therapy session with us and bring them to your first appointment.  Read all of the material and complete the forms to the best of your ability.  Leave blank any areas you may need help with or have quesitons on.  The forms are intended to provide your therapist with accurate information about your overall health.

Your First Visit

For your first visit, please arrive at least 15 minutes early to be registered into our computer system.  Please bring the following with you.
     1.  The filled out forms
     2.  Your prescription for therapy from your doctor
     3.  Your insurance card
     4.  A photo ID
     5.  Your appointment calendar to schedule follow up appointments
 
Please wear comfortable clothes and shoes.  The therapist typically spends 30 minutes to 1 hour completing your initial evaluation.

Rehabilitation Services Questions & Answers

Where is Mason District Hospital Rehabilitation Services Department located?
A. Outpatient Therapy (PT, OT, SLP) – Mason District Hospital, adjacent to the Cardiac Rehab Department. Inpatient Therapy (PT, OT, SLP) Mason District Hospital Acute Medical Surgerical Floor to the left of the main lobby.

What are the hours of operation?
A. Mason District Hospital Outpatient PT, OT, SLP services are available Monday through Friday, 7:30 am to 4:30 pm. Inpatient Rehabilitation Services are offered Monday through Friday and one time during the weekend either Saturday or Sunday.

How can someone get therapy services?
A. Outpatients should call or stop in to make an appointment (bring physician prescription). Inpatient services will be specifically ordered by your physician.

How soon can someone get therapy services?
A. Outpatients can typically be scheduled for an appointment within 2 – days or sooner as available. Inpatients are seen by the therapists within 24 hours of receiving a physician referral.

What makes Mason District Hospital Rehabilitation Services different from other area Rehab facilities?
A. Mason District Hospital is one of the few local rehab departments where PT, OT, and SLP may be provided in the same location. Mason District Hospital Rehabilitation Services Department houses a NeuroCom Smart Balance machine, Baltimore Therapeutic Exercise machine, and many other state of the art pieces of exercise equipment in our newly renovated space. We also offer private assessment rooms for your therapy initial evaluation and follow up therapy treatment sessions. Additionally, as a hospital-based rehab department, emergency and other services are immediately available as needed.
